How do I get an appointment?
Simply call SMRMC’s Center for Physical Rehabilitation at 753-3070 and tell them that you are interested in the Wheelchair Clinic services. A physical therapist will contact you to gather more information. You will be scheduled for a thorough physical therapy evaluation.

7:00 am to 4:30 pm, Tuesday through Friday
(Appointments outside these hours are available upon request)

Who will be at the evaluation?
A physical therapist will perform the physical evaluation. The evaluation will take approximately 1-2 hours. After the physical therapy evaluation the physical therapist will contact a wheelchair vendor, inform them of evaluation results and give the vendor your contact information.  The vendor will come to your house and show you the wheelchair options appropriate for you.

What if my chair/cushion is not covered by my insurance company?
If your chair is denied coverage through your insurance company, you will be given information by the therapist and/or vendor about other avenues of coverage. These may include community service organizations, grants, etc. The last resort would be for you to pay directly for your chair. You may or may not choose to do this; this should not affect whether or not you come to the Clinic for an evaluation.

How long before my new chair arrives?
The approval process can be lengthy.  Depending on what type of insurance you have. It could be up to 4 months before you get your chair.
