To sign-up for this service, when you next visit your primary care provider's practice, please tell the receptionist you would like to use Your HealthLink. The receptionist will ask you to sign a consent form, and then give you a Personal Identification Number (PIN). This PIN is necessary to complete the online registration process. A "welcome" e-mail will then be sent by the practice to your home
e-mail. This "welcome" e-mail will help you take the next steps in using Your HealthLink. We do not provide this information over the phone to ensure your privacy and the security of your medical records. Hope you will register.
