Memory Clinic


Community Clinical Services,affiliated with St. Mary’s Health System, offers a Memory Clinic with an interdisciplinary service team.  Our Memory Team focuses on the assessment and treatment of cognitive decline in individuals over 65 years old and older, as well as the mood and behavior changes that may accompany dementia. They also provide nursing home and residential care visitation,  currently with St. Mary’s d’Youville Pavilion, Clover Manor, Schooner Estates, and the Woodlands.

Our Memory Team:
Patients and family members will meet with: Psychiatrist Noreen Flanagan, MD, Licensed Clinical Social Worker, Elizabeth Richards, LCSW, and Registered Nurse, Sue Cayer, RN, to assess the needs of the patient.

NOREEN FLANAGAN, MD 
Dr. Flanagan is a board certified psychiatrist and the Director of Geriatrics for CCS Psychiatry. She received her undergraduate and graduate degrees from Boston University, earned her medical degree from the University of Vermont, and trained in Psychiatry (geriatric and dementia care) at Maine Medical Center in Portland. 

ELIZABETH RICHARDS, LCSW 
Elizabeth is a licensed clinical social worker with a Master Degree from the University of New England, and a Certificate in Gerontology and Bachelor degree in Social and Rehabilitation Services from Assumption College.  Elizabeth has worked as a psychiatric social worker, and as a Director of Social Services in a nursing home setting.  She specializes in neuro-psychological testing, and counseling individuals and families with recent diagnosis of cognitive impairment. 

SUE CAYER, RN 
Sue is a Registered Nurse with over 30 years of experience in Long Term Care and Behavioral Nursing for those with cognitive impairment or mental illness. She has worked as a Director of Admissions and Marketing, Nursing Director, and a Clinical Resource Nurse in Behavioral Health.

Our team will:

  • work closely with primary care providers and other specialists as needed to identify factors that may be contributing to problems with thinking. 
  • utilize medication and non-medication intervention options that include strategies for safe functioning in the individual’s current living environment. 
  • connect families to area resources including Seniors Plus and the Alzheimer’s Association. 

Through early diagnosis and treatment of cognitive decline, they will work to help individuals stay in their home environment for as long as possible while helping to support the individual, family and caregivers. 

Consult and Follow-Up Appointments
For the consult appointment, patients and families can expect to meet with Sue Cayer, RN for the first 30 minutes for a comprehensive nurse assessment.  Sue will then introduce the patient and family to Elizabeth Richards, LCSW who will use the next 30 minutes to administer the Montreal Cognitive Assessment (MoCA) Neuropsychiatric Test, Geriatric Depression Scale and screen for any concerns related to driving.  Elizabeth will follow-up with the family on any referral needs identified from the nursing assessment.  Elizabeth will then introduce the family to Dr. Flanagan who will meet with the patient and family the last 30 minutes  for brief pertinent neuropsychiatric testing, neurological exam, mental status exam, discussion of MoCA results, review of medication options and any further work-up recommendations (imaging, lab work, neurology referral, etc.) Sue and Elizabeth remain available to the patient and family during this last 30 minutes for additional support and to answer any questions or concerns.
