Directions to the Center for Senior Medicine
The Center for Senior Medicine is A Federally Qualified Health Center and a full-service geriatric medicine practice.
Geriatric medicine specializes in the medical needs of people who are 65 years of age and older. The Center for Senior Medicine has extensive experience caring for people with Alzheimer's disease, working in long- term care facilities (nursing homes), and providing consultation to other physicians on unique problems associated with aging. Because our team represents a variety of disciplines, we bring a unique understanding of the ways age impacts chronic health conditions.
The Center's team includes medical, nursing, and social work professionals. Karim Yacoub, MD, is the Center's geriatrician and Nan Boucher is the Adult Nurse Practitioner and Geriatric Nurse Practitioner. The Center for Senior Medicine is managed by Renée L'Italien, and Pam Morin is the Office Coordinator.
