The Center for Transitional Rehab Care (CTRC) provides skilled nursing and rehab services to patients recovering from surgery, an illness, or injury.
Care is provided under the direction of your attending physician, and by an interdisciplinary team. The primary goal for the CTRC is to assist you in your recovery and return to your maximum level of independence. Our purpose is to fill the gap between your hospital stay and home care. Often, after time spent in the hospital recovering from an illness or injury, you may require additional therapy or education related to specific illnesses, care, or medications before going home. Our staff offers the attention and support you need in a rehabilitative setting. Here you, your physician, therapists, nursing staff, social services, and the Diet Office work together to meet the challenges that frequently accompany illness and or injuries. Our goal is to help prepare you to return home safely as soon as possible.
Integrated treatment programs include but are not limited to:
- Oncology care
- Postoperative care
- Dialysis support
- Pulmonary management
- Respiratory treatment
- Physical and Occupational Therapies;
- gait training
- activities of daily living
- home evaluations
- kitchen management
- balance training
- cognitive therapy
- wheelchair assessments
- Speech Therapy (includling language and swallowing)
- Skin Care

- Primary care team nursing
- Diabetes management
