The Transitional Life Care (TLC) Unit provides skilled nursing and rehab services topatients recovering from an illness or injury and cannot immediately return to their home. Care is provided under the direction of the patients attending physician and by an interdisciplinary team lead by the Unit's Medical Director, a certified geriatrician. The primary goal for the TLC unit is to help patients recover and return to their maximum independence. The 42 bed Unit typically provides short-term care for those recovering from a hospital stay or injury. Integrated treatment programs on the Unit can include:
