The Outpatient Diabetes
Education Program
1. Physician
Referral
Please have your doctor call the Diabetes Education Program to
refer you to our program at (207) 777-8625.
2. First Visit
You will meet with the diabetes educator to begin learning about
diabetes. We will talk about the importance of blood sugar testing.
3. Dietary Visit
Learning about nutrition is the most important step in your new
lifestyle. You will meet with a dietitian to develop a meal plan
that fits your lifestyle.
4. Diabetes Education Classes
One day class will teach you what you need to know about diabetes.
5. Visit after Classes are Complete
You will be scheduled to meet with the diabetes educator when
the classes are finished. At regular periods after classes are
done, you'll see the nurse to help you manage your diabetes yourself.
Finally, one year later, you and the rest of your class will get
together to see how you all are doing and to review what you have
learned.
Insurance
Coverage Information
Diabetes education classes are covered by most major insurance carriers.
Please check with your carrier to make sure you are covered.
Other Services
The Diabetes Education Program also does insulin management.
Individual services are provided for child, teen and gestational
diabetes.
