IMPORTANT: In May, we will be introducing our new CareLink Electronic Health Record and its improved patient portal, called MyChart.
MyChart will only have clinical data from May 2017 onwards. If you want to download your clinical data prior to May 2017, follow the instructions below. This must be done before May 2018, as the current patient portal will unavailable after this point. We recommend that that you only download this data to your own private computer or device and avoid using a public Wi-Fi network.
- Log onto your account using the links below.
- Click "View Your Chart Summary" under Select an Option
- Click “Chart Summary” from menu on the left under Personal Information
- Click “Download or Print" button in the upper right corner
Medical records may also be requested through the Medical Records Department.
